 Well, you just need to make sure that the person taking your child is in possession of Travel Consent Document. Here is the sample:

Travel Consent

(parent name)  make oath and say that I
am the lawful Father of __________________ (child name) (Female) born on _________ in ____________, passport, number ____________, issued on _______________ in _____________;  .



 



2. ________________ (name of child) (my child) has my
